Patient Care Team

The Patient Care Team works to provide services to assist patients with accessing resources, providing education and advocating to improve the overall physical and emotional wellbeing of each person.  Services provided by the Patient Care Team include Care Coordination and Integrated Behavioral Health.

To address issues of social isolation and access to behavioral healthcare in our community, Kearny County Hospital has placed a high priority on mental and behavioral health services with our Integrated Behavioral Health Clinic.

The program integrates behavioral health into our primary care model, with masters-level social workers on hand to offer mental health and wellness screenings for patients who report or exhibit signs of depression, anxiety, chronic conditions and/or social difficulties. As it aims to overcome the stigma of mental health issues.

Our team supports patients and family members as they engage the healthcare system on any level, including hospital admissions, emergency room encounters, and outpatient clinics.  Our aim to is ensure patients are empowered to navigate the best course of care to promote for their personal health and wellness.

As part of these services, our Mental Health First Aid for Youth course offers guidance in assisting young people who are experiencing mental health-related issues. Participants learn the risk factors and warning signs of mental health challenges; strategies for handling crisis and non-crisis situations; and where to turn for help. Kearny County Hospital makes this training available in the surrounding area.
